The SFAR Foundation's Welcome Home Program (WHP) partners with a network of dedicated community-based organizations, civic leaders, and sponsors to provide formerly homeless individuals and families with much needed new household items in order to help them settle into safe and healthy permanent housing.
We request this additional demographic information regarding your client so that WHP can better serve your clients. Such demographic data is also often required when WHP applies for funding from potential sponsors and philanthropic donors. Your agreement to provide this basic demographic data is voluntary. The information provided will in no way be used for determining whether your client is eligible for assistance from WHP, and WHP will not share this data with third parties as individual points of data. But data may be provided in aggregate, anonymous (no names attached) format to comply with the requirements of potential philanthropic organization from which WHP seeks support.
By voluntarily providing WHP with the information on behalf of your clients, you and your clients consent to WHP so using the information and waive any claims against WHP or any such philanthropic organization arising from or relating to such use of and access to the information. Again, voluntarily providing this data will help our ongoing efforts to sustain and improve the WHP, and potentially expands support for this vital program.
